The Prism Awards honor individuals and organizations that have demonstrated exemplary achievements in the areas of:

  • -Diversity, Equity, Inclusion and Belonging (DEIB): This award honors individuals who have championed DEIB in ways that have resulted in demonstrable progress within their companies and/or their respective industry

  • -Environment Sustainability: The Green Guardian Award recognizes organizations that have made significant achievements in environmental sustainability

  • -Social Stewardship: This award spotlights companies that have made significant achievements in social responsibility initiatives related to community engagement, such as philanthropic efforts or contributions made to the community through programs, partnerships and outreach

All entries are $500 USD through November 30, 2024. Payment method is by credit card only. Failure to pay the entry fee will prohibit consideration for the application to the Prism Awards program.

Submissions for the 2024 Prism Awards open September 5, 2024. Entries will be accepted until November 30, 2024 at 11:59pm CT. Winners will be announced January 15, 2025.

Yes. We will accept a maximum of four (4) nominations for individuals from a single organization. Once the limit is achieved, the Awards Program Manager will alert the organization that the maximum number of submissions has already been reached for the annual program.

The Prism Awards application process requires all the following components and responses as part of the submission:

  • -Diversity, Equity, Inclusion and Belonging (DEIB) Award (Recognizing individuals)
    -Nominee and nominator’s name, title, company, and contact information -Current resume or CV (Word or PDF format) -Summary of achievements: Submit a 500-word or less statement that outlines the nominee’s role within the organization, including whether driving DEIB initiatives is explicitly a part of their responsibilities, and a summary of achievements as to how their efforts have positively influenced their teams, company, industry and/or community. Share metrics that illustrate the nominee’s impact. -Letters of support: Include at least two (2) letters or a maximum of four (4) letters of support (at least one (1) letter must be external to the nominee’s organization) that provides testimony about the nominee’s efforts and accomplishments related to DEIB. Supporting materials: These are optional and can include anything to help showcase the work that the nominee has accomplished in the realm of DEIB, including: websites, statistics, media coverage, research results, documents, photos, and/or videos.

  • -Green Guardian Award (Company recognition of an initiative completed between January 1, 2023 - June 30, 2024)
    -Company description: Provide company name, size, primary services delivered, headquarters and number of locations, and names, titles and contact information of those involved with the initiative. -Summary of achievements: Provide a description of how the organization has pursued environmental sustainability and what results those efforts have achieved. Share key metrics or other tangible evidence of the scope of those accomplishments. -Letters of support: Include at least two (2) or a maximum of four (4) letters of support from either individuals or organizations involved in or affected by the company’s sustainability initiatives. -Supporting materials: These are optional and can include anything to help showcase the work that the nominee has done in the realm of environmental sustainability, including: websites, statistics, media coverage, research results, documents, photos, and/or videos.

  • -Social Stewardship Award (Company recognition of an initiative completed between January 1, 2023 - June 30, 2024)
    -Company description: Provide company name, size, primary services delivered, headquarters and number of locations, and names, titles and contact information of those involved with the initiative. -Summary of achievements: Provide a description of how the organization has pursued social responsibility initiatives related to community engagement, such as involvement in philanthropic efforts or contributions made to the community through programs, partnerships and outreach. Describe what those efforts achieved. -Letters of support: Include at least two (2) or a maximum of four (4) letters of support from either individuals or organizations involved in or affected by the company’s social stewardship initiatives. -Supporting materials: These are optional and can include anything to help showcase the work that the nominee has done in the realm of social stewardship, including: websites, statistics, media coverage, research results, documents, photos and/or videos.

WTWH Healthcare will complete an initial screening of applications, then a panel of independent judges will select the inaugural Prism Awards Class of 2024.

TERMS & CONDITIONS

The Prism Awards (“Awards”) take place between September 5, 2024 and November 30, 2024. The Awards start with a nomination process in which individuals or organizations will nominate an individual or an organization to be considered for the program. There are three awards under this program, including:

  • Diversity, Equity, Inclusion & Belonging (DEIB) Champion Award
  • Green Guardian Award
  • Social Stewardship Award

A panel of industry judges will evaluate each entry in its respective category to determine the winners of the 2024 Prism Awards.


To be eligible, individual and organizational nominees must represent one of the following industries:

  • Behavioral Health
  • Complex Rehab Technology (CRT)
  • Home Health & Home Care
  • Home Medical Equipment (HME)
  • Hospice & Palliative Care
  • Memory Care
  • Senior Housing & Senior Living
  • Skilled Nursing

Each submission may choose from one of the following categories:

DEIB Champion Award: Honors individuals who have championed DEIB in ways that have resulted in demonstrable progress within their companies and/or their industry.

Green Guardian Award: Recognizes companies that have made significant achievements in environmental sustainability.

Social Stewardship Award: Recognizes companies that have made significant achievements in social responsibility initiatives related to community engagement, such as involvement in philanthropic efforts or contributions made to the community through programs, partnerships and outreach.

Each award entry fee must be paid prior to the entry deadline of November 30, 2024 to be considered for eligibility. Entry fees are non-refundable. Prior award winners should not be resubmitted for nomination to the program.


Individuals or an organization may submit a nominee for the Awards program via the online submission form starting September 5, 2024 and complete the process prior to 11:59 PM Central Standard Time on November 30, 2024 for consideration. Upon submission, each entry will be entered into the Awards. Each nominee accepts these terms and conditions upon accepting the nomination hereby agrees to the terms and conditions of the Awards. By entering the Awards, entrants and nominees agree to consent to WTWH Healthcare, a WTWH Media company, using the personal information for the purposes of evaluating, judging and promoting as part of agreement to participation in the program. Failure to claim or accept the nomination or meeting the eligibility criteria may result in disqualification or forfeiture of the participation in the Prism Awards.

      • Provide a written statement that describes how the company has pursued environmental sustainability and what results those efforts achieved. Share key metrics or other tangible evidence of the company’s accomplishments (PDF or Word)
    • Social Stewardship Award
      • Provide a 500-word or less written statement that describes the company’s information, including name, size and primary services delivered (PDF or Word)
      • Provide a written statement that describes how the company has pursued social responsibility initiatives related to community engagement, such as involvement in philanthropic efforts or contributions made to the community through programs, partnerships and outreach. Share key metrics of what those results achieved (PDF or Word)
      • Provide one (1) to three (3) letters of support from individuals or organizations involved in or affected by the company’s social stewardship initiatives (PDF or Word)
      • Optional: Include any websites, statistics, media coverage, research results, documents, videos and/or photos showcasing the nominee’s Social Stewardship impacts (PDF or Word)
      • Any additional information for the judges


The entry fee for the 2024 Prism Awards is $500. All submissions must be received by 11:59pm Central Standard Time on November 30, 2024.

All submissions will be evaluated by a panel of industry experts. Selection of a Prism Award winner is at the sole discretion of the aforementioned panel of judges. The entries selected for the 2024 Prism Awards for each category will be announced online in January 2025.

In such an event, WTWH Healthcare, a WTWH Media company, reserves the right to award the prizes from among the eligible entries or plays received up to the time of the impairment. WTWH Healthcare, a WTWH Media company, reserves the right, in its sole discretion, to disqualify any individual it finds to be tampering with the entry process or the operation of the or to be acting in violation of the Awards terms and conditions of this or any other promotion or disruptive manner. Any attempt by any person to deliberately undermine the legitimate operation of the Awards may be a violation of criminal and civil law, and, should such an attempt be made, WTWH Healthcare, a WTWH Media company, reserves the right to seek damages from any such person to the fullest extent permitted by law. WTWH Healthcare’s, a WTWH Media company, failure to enforce any term of these Official Rules shall not constitute a waiver of that provision.

By submitting nomination, nominees (or persons entering on behalf of nominee) shall allow WTWH Healthcare, a WTWH Media company, and/or WTWH Healthcare, a WTWH Media company, designees the perpetual right to use his/her entry, name, biographical information, photos, videos and/or likeness, and statements for promotion, trade, commercial, advertising and publicity purposes, at any time or times, in all media now known or hereafter discovered, worldwide, including but not limited to on the World Wide Web and Internet, without notice, review or approval and without additional compensation except where prohibited by law. A signed affidavit of eligibility, publicity release and release of liability may be required.


Issues concerning the construction, validity, interpretation and enforceability of these Official Rules shall be governed by the laws of Illinois.


By participating, each nominee releases and agrees to indemnify and hold harmless WTWH Healthcare, a WTWH Media company, & Others from and against any and all costs, claims, damages, or any other injury, whether due to negligence or otherwise, to person(s) or property (including, without limitation, death or violation of any personal rights, such as violation of right of publicity/privacy, libel, or slander), due in whole or in part, directly or indirectly, to participation in the Awards.


WTWH Healthcare, a WTWH Media company, reserves the sole right to amend or alter the terms and conditions of the Awards at any time. By entering the Awards, the nominee agrees to abide by these Official Rules and the decisions of WTWH Healthcare, a WTWH Media company, which shall be final in all matters pertaining to this Awards program. All entries become the property of WTWH Healthcare, a WTWH Media company, and will not be returned. Void outside of the fifty (50) United States/District of Columbia and wherever prohibited or restricted by law. All federal, state and local laws and regulations apply.
